  1. Accomplished Application Form
  2. Scanned copy of (any of the following):
  • for Corporation – Security and Exchange Commission (SEC), Articles of Incorporation, By-Laws & General Information Sheet
  • for Single Proprietorship – Dept. of Trade & Industry (DTI)
  • for Cooperative – CDA Registration
  • for Barangay Micro Business Enterprise (BMBE) – BMBE Certificate
  1. Updated BIR Certificate of Registration Form No. 2303
  2. Latest Mayor’s Permit
  3. Latest Audited Financial Statement or Interim Balance Sheet (for newly established companies)
  4. One (1) 2”x2” Colored Picture of Authorized Representative/Owner
  5. Not included in the DTI Watchlist
  6. Photos of Office (Inside & Outside), Wall-posted Permits and Products using the application GPS MAP CAMERA
  7. Application & Entrance fee of P1,000.00 only / Annual dues of P3,600.00 only
  • All applications are subject of Board Approval
